Investigation Summary
At approximately 10:00 a.m. on September 7, 2003, Employees #1 and #2 were working in the bottom of an unguarded trench to install a plastic storm drain that was 24 inches in diameter. At the top of the trench, it measured 18.5-feet deep by 50-feet long by 28-feet wide and 2.5-feet wide at the bottom. For some reason, the trench collapsed, trapping Employees #1 and #2. Coworkers jumped into the trench in an attempt to rescue the two men, but were unsuccessful. Approximately 15 to 20 minutes later, they were ordered out of the trench by the local police. The Urban Search & Rescue Team (Fire Department) pulled Employees #1 and #2 from the trench. Employee #1 sustained minor injuries, which he was treated for at an area hospital. Employee #2 was transported, by helicopter, to the Shock Trauma Unit in Baltimore, Maryland where he was admitted and treated for multiple fractures on both legs, fractured ribs, and other minor injuries. An investigation determined that both men were not wearing personal protective equipment and there was no ladder or other safe means of egress from the trench in place. In addition, there was no trench box available on site.
