Employee Is Killed in Fall from Elevation

Employee #1 was working with a coworker to reinstall a piece of fascia aluminum which had blown down during a recent hurricane. The piece was to be reinstalled on the fascia of the second story of a building at a self-storage facility, at a height of approximately 21 ft above ground. Employee #1 was on a 24-ft extension ladder and he was reaching above his head and backwards when he lost his balance and fell approximately 15 ft to the paved ground. Employee #1 struck his head on the ground which caused his death. The coworker was on the ground at the time with his back to Employee #1 and did not see the actual fall.
