Investigation Summary
An employee had placed a tarp on the roof of a home that was damaged from a hurricane. While descending a ladder, his foot slipped through the rungs on the ladder and he lost his balance. He fell backwards, approximately five or six feet, and struck his head on the concrete driveway. He died as a result of his injuries.
