Employee's finger amputated by running motor

On May 24, 2004, Employee #1 was working with coworkers to repair a high-lift water pump. After replacing the motor on the water pump, Employee #1 fell into an unguarded motor (US Motors 7 1/5 hp) mounted vertically on a high-lift jet pump. His right little finger was amputated and his right ring finger was lacerated. Employee #1 was not hospitalized. The motor had an anti-reverse mechanism mounted on the top end of the motor. At the time of the accident, the employee and coworkers were testing the system and the guard for the anti-rotation portion of the motor had not been replaced.
