Investigation Summary

Investigation Nr: 200528578
Event: 04/12/2004
Employee's Finger Is Amputated by Board Stacker

On April 12, 2004, Employee #1 was operating the 4600 BS Board Stacker. The boards, stringers, travel through different stages of this stacker ending up being automatically stacked on a skid. The stages are to align the correct number of stringers together to be placed on each level of a stack. At the stage where the correct number of stringers is aligned is where the accident occurred. While traveling through this stage, a single stringer was not properly lined up and was positioned laying flat because it has to be positioned on its side. Employee #1 reached in to position the stringer correctly with his left arm and hand. The left little finger came in contact with either the cycling pusher or the board and pressure plate as the pusher applied pressure to position the stringer correctly. The finger was pinched or squeezed, removing the skin on the palm side of the little finger exposing the bone from the tip back approximately one inch. The finger nail was still intact. A doctor amputated the finger back to the middle knuckle. The employee was not hospitalized.
