Employee's index finger amputated

On December 23, 2003, Employee #1 was using a HYD-MECH, 11 Series, Model Number: S-20A, band saw to cut a piece of steel round stock. The stock was approximately 2.5 inches in diameter and 9-inches long. Additionally, the band saw could be operated on either manual or automatic mode. As Employee #1 had the saw in manual mode, he reached to the top front of the saw, with his left hand, to activate the hydraulic vise. The vise activated while Employee #1 still had his right hand placed on the stock in the vise area. The vise moved forward, crushing his right index finger between the moving vise and the round stock. Employee #1's right index finger was amputated at the middle knuckle.
