Investigation Summary
On June 23, 2004, an employee was driving a collector truck. The collector truck plate began to move at the top of a slope in the road. It suddenly developed mechanical problems with the acceleration and brake system. The employee lost control. He turned the truck toward the road wall and the two coworkers in the back of the truck were thrown into the air. The employee was killed.
