Employee falls from extension ladder and injured

On August 18, 2003, an employee was masking an exterior window in preparation for painting of house. He was standing on a 24-ft Aluminum Extension Ladder that was not extended and was working at 10 to 12-ft height level. As the employee leaned or reached beyond arms reach, ladder apparently shifted to the right and fell. Ladder landed on the ground with the side rail facing up and the employee fell on top of the ladder striking his head on the ladder rail. He was hospitalized and treated for dislocation.
