Employee is killed in fall from ladder

On June 4, 2003, Employee #1, a siding installer, was stapling sheeting to the side of a new apartment complex using a 30-ft extension ladder. He had set the ladder up on a stack of plywood and the plywood slid out from under the ladder while the employee was in his working position. Employee #1 fell approximately 24 ft from the ladder. Employee #1 suffered fractures and was killed.
