Investigation Summary
At 10:30 p.m. on May 30, 2003, Employee #1 was in the process of reinstalling a rubber strip through a conveyor system onto a retread tire machine. As he was threading it through and over a moving pulley, Employee #1 slipped. His left arm landed on the pulley and pinched his muscle. Employee #1 sustained a deep laceration to his muscle requiring 14 staples and an overnight hospital stay.
