Employee Killed When Caught Between Forklift and Panels

Employee #1 was using a forklift to place a 16-foot by 5-foot panel on top of a bundle of panels that were already in the yard. He got off of the forklift to push the panel onto the top of the stack that was already in place. Employee #1 passed beneath the elevated forks. The forklift rolled forward, trapping him between the forklift and the stationary stack of panels. Employee #1 died from asphyxia.
