Investigation Summary
On October 18, 2003, Employee #1, a welder's helper, was holding steel plates in place for a coworker. They were using a scaffold but working in an enclosed space. At approximately 5:45 p.m., they took a break. The coworker left the space but Employee #1 stayed on the scaffold and lay down. At approximately 6:05 p.m., the coworker returned and found Employee #1 unconscious and without a pulse. He was transported to Halifax Memorial Hospital, where he was pronounced dead of apparent heart failure.
