Employee killed in fall from conveyor system

On April 19, 2004, Employee #1 was scrapping structural elements of a grain handling conveyor system to prepare them for washing and painting. Employee #1 fell approximately 90 ft and was killed. Employee #1 was provided with a fall protection and prior the accident he was seen utilizing it.
