Employee dies due to burns received from flash fire

On August 26, 2003, Employee #1 was cleaning a temporary filter. For some reason, the filter exploded. A flash fire occurred and Employee #1 was engulfed and severely burned. Emergency services were contacted and Employee #1 was transported by helicopter to Herman Hospital, where he was hospitalized. He died 25 days later on September 20, 2003.
