EMPLOYEE #1 WAS PRESSURIZING A TIRE TO SEAT THE BEAD ON THE RIM WHEN THE RIM EXPLODED. EMPLOYEE #1 WAS KILLED WHEN HE WAS STRUCK IN THE FOREHEAD BY FLYING PIECES OF THE EXPLODING RIM.
