Investigation Summary
At approximately 12:38 p.m. on June 24, 2003, Employee #1, of Drees Homes Construction, set up a 20-ft base section of a Werner D-1240 40-ft aluminum extension ladder against a two-story the garage that was under construction. One swivel shoe of the ladder was missing the base rubber pad. Employee #1 set the ladder against the gutter with the base approximately 6 feet from the garage wall. The employee turned up the broken swivel shoe and set the ladder frame into a crevice created by an expansion joint in the concrete sidewalk. The other ladder swivel shoe rested on the asphalt driveway. Employee #1 then climbed the ladder and hooked the handle of a three-gallon paint can over the ladder end. While the employee was reaching out to paint a shutter, the ladder base kicked out from the crevice in the concrete propelling Employee #1 onto the garage roof. The employee then fell headfirst onto the asphalt driveway. Employee #1 sustained serious head injuries and was transported to the Shock-Trauma Unit at INOVA Fairfax Hospital. The employee died two days later.
