Investigation Summary
On October 9, 2003, Employee #1, a farm employee, was assisting a tire company representative in changing a 3,000-lb tractor tire. The tractor was elevated with jacks and had no support such as cribbing. When one tire was removed, the tire company representative noticed that the tractor began to lean due to failure of the jack. Employee #1 tried to hold up the tire, however, it fell, crushing him, and killing him.
