Investigation Summary
On June 25, 2003, several employees were riding in and on the cab of a frontend loader. The driver was a crew leader. After getting a soda, the driver started to drive the front- end loader with two employees in the cab, Employee #1 was on the ladder and told the driver that he was find riding on the ladder. The driver hit some small bumps in the road and Employee #1 lost his balance and was crushed by the rear wheels of the frontend loader. He died of his injuries.
