Investigation Summary
On July 30, 2003, Employee #1 was knocking down a hung-up tree by falling another tree to it. The second tree did not knock down the first tree, but instead slid down the first tree and struck Employee #1 pinning him to the ground. Employee #1 was found dead with the tree lying on the back of his head.
