Investigation Summary
On April 29, 2003, Employee #1 was attempting to tie off a 24-foot extension ladder to the metal roof of a building. The base of the ladder was resting on a polyethylene plastic, which covered the entire asphalt parking area. As Employee #1 stepped onto the roof, the ladder base slipped causing him to fall 9.66 feet to the pavement. Emergency services were contacted, who stabilized and transported Employee #1 to the hospital. He was hospitalized and treated for a hair-line fracture to his spine. Employee #1 was released the same day.
