Investigation Summary

Investigation Nr: 201088549
Event: 09/18/2006
Employee Dislocates Foot When Struck by Forklift

At approximately 10:00 p.m. on September 18, 2006, an employee was operating a forklift. The worker's electric Raymond Model Number R40 standup forklift was parked inside the warehouse area as a coworker drove up to his side in another identical forklift. The employee had begun to step out from the operator's station with his left foot. As the coworker began to step out, he had his right foot on the dead-man pedal and hit the propulsion lever, causing the lift to collide with the employee's lift catching his left foot underneath and causing an injury to his foot. The employee was transported to the University Medical Center in Fresno. He was hospitalized and treated for foot dislocation.
