Investigation Summary
At approximately 10:30 a.m. on October 7, 2005, Employee #1 operating an empty Caterpillar earthmover Model 631 was injured in a collision with another earthmover. Employee #1 was operating on a private roadway. He had just started his shift when he was crossing over a two-lane roadway and a loaded Caterpillar Model 631 earthmover came over the hill and collided with Employee #1's empty earthmover. The loaded 631 earthmover struck Employee's #1 earthmover and caused it to roll over. Employee #1 was hospitalized for a fractured leg, head trauma and was in a coma for five days.
