Employee killed in fall from ladder

Employee #1 was instructed to set up a stepladder on the third base concourse so another employee could repair a sign frame that had been damaged by the wind. He then climbed the ladder and was on the fourth step, balanced on one foot and hanging onto the sign frame, when he fell back, landing first on his posterior and then hitting his head. He was killed in the fall.
