Investigation Summary
At approximately 11:28 a.m. on April 11, 2003, a crew was removing wall forms from the east side of the gymnasium locker room under construction. The top section of a form, that was being lifted away by a crane, fell off and struck Employee #1. He was transported to Hilo Hospital and transferred to Queens Hospital where he died as a result of his injuries.
