Investigation Summary
On March 31, 2003, a crew was placing a new guardrail system on a four lane divided highway. At the time of the incident, the guardrail was being placed on a bridge overpass section along with the existing railing. Employee #1 and Employee #2 were bolting a previously placed guardrail to a post. They used a F750 Ford Truck to drive the guardrail posts into the ground and pull the post out of the ground. The operator was asked by someone to back up the truck. The truck has two operator stations, one at the cab and the other at the rear of the vehicle. A section of guardrail was rigged by a chain to the truck's post puller attachment for the purpose of handling the guardrail section and splicing it to the guardrail previously placed upon the posts. The operator reported that the emergency brake hand control was on, and the brake foot pedal depressed, as reverse was selected on the transmission control while positioned at the rear operator station. The operator reported that when reverse was selected, the truck's engine operated at a rate at, or near, wide open. As a result, the truck traveled in reverse out of control. Evidently, while the truck was traveling, the guardrail section rigged to the post puller became lodged against an installed guardrail post, subsequently became dislodged and swung into Employee #1 and Employee #2. Employee #1 was pronounced dead at the scene. Employee #2 died within 24 hours at the hospital. Evaluation of the truck did not reveal conditions or situations as to why it traveled in reverse at an unexpectedly high rate of speed. The investigation concluded that the operator may have inadvertently depressed the accelerator pedal as opposed to the brake pedal.
