Investigation Summary
On April 3, 2003, Employee #1 was accessing the roof of an office building to conduct service on heating, ventilation, and air conditioning units. Employee #1 was utilizing a fixed folding ladder, which accessed a roof hatch. Employee #1 was using the ladder with the base only 19 in. away from the wall. The ladder could fold out to 70 in., providing a 70-degree ladder angle. Employee #1 was holding a 60-lb cylinder of compressed nitrogen on his shoulder with his right hand while climbing. He used his other hand to grab the rungs. As Employee #1 reached for the last rung he missed it and fell 16 ft to the platform. Employee #1 landed on his feet first, then fell back on his head. Employee #1 sustained a concussion and head lacerations requiring seven staples and ten sutures. He was held overnight in the hospital due to dizziness and nausea.
