Investigation Summary
On February 6, 2003, Employee #1 was utilizing an industrial lift truck to load drywall onto a flat bed delivery truck. The drywall was to being delivered to a job site the next day. The loading process requires the placement of spacers (dunnage) on the delivery truck bed to raise the load for access during unloading at the delivery site. The employee was working alone and left the lift truck unattended to step between the load and the trucks' bed to position the spacers (set dunnage). While he was between the load and the truck bed, the lift truck apparently rolled forward into a low spot in the terrain and trapped the employee. The employee was found by a coworker at approximately 9:00 p.m. Employee #1 was killed.
