Investigation Summary

Investigation Nr: 201281680
Event: 01/21/2004
Construction Worker Killed in Fall from Scaffold or Ladder

On or about January 21, 2004, Employee #1, an employee of DE/TE Builders, Inc., was performing concrete finishing work (punch list) when he possibly fell from a mobile scaffold and/or aluminum ladder which was used to access the scaffold which were both measured at a height of 7 ft above concrete ground level. There were no eyewitnesses to the accident. Two other employees walking by the area discovered Employee #1 lying unconscious on the concrete ground next to the fallen 7-ft ladder with his left lower leg lying across the second and/or third rung. Also, a 7 ft by 5 ft mobile scaffold from which Employee #1 was working was in the area, still in an erect position next to the fallen ladder and Employee #1. Emergency Medical Services were summoned, and Employee #1 was taken to Macon Medical Center, where he died approximately a week later on January 26, 2004. According to Employee #1's son an autopsy will not be performed. Also, Employee #1's son stated that the neurologist of the Macon Medical Center ruled out heart attack and stroke. While Employee #1 was in the hospital, extensive tests were performed, and he did not have any medical history. Therefore, the neurologist ruled the cause of death as blunt trauma to the head relating to accident.
