Investigation Summary
On December 17, 2003, Employee #1 and four coworkers were attempting to replace part of a water line and were working in a 16-ft-deep trench. The employees were repositioning the double trench box system. After placing the first trench into the trench, Employee #1 positioned himself within the trench to receive the second trench box, so to connect it to the first. As he inserted the connecting bolts into the first box to connect the two boxes together, a spoil pile above the trench collapsed prior to the second box being placed in the trench. The trench collapsed and buried him, resulting in his asphyxia.
