Investigation Summary
Employee #1 was returning a heavy capacity forklift to an employer on the other side of an airport after receiving a request to have it returned. Employee #1 proceeded toward the other end of the airport by driving the forklift on the service road (a public way) instead of traveling around the terminal areas. A coworker followed Employee #1 in a chase vehicle. As Employee #1 was traveling down the road, the mast of the forklift struck an overpass. The impact of the collision bent the mast backward toward Employee #1. The coworker jumped on the moving forklift and stopped the vehicle. Employee #1 died at the scene.
