Investigation Summary
On May 7, 2003, Employee #1 was using a torch to remove the cover from a hydraulic arm that had been brought into the machine shop for repair. The heat from the torch caused the cover of hydraulic arm to blow off and the hydraulic fluid caught fire. The employee was burned and killed.
