Investigation Summary
On February 10, 2003, Employee #1 and his coworker were installing a new belt on a conveyor. The belt was moving to one side, and they were adjusting the alignment of the conveyor. A piece of wood got caught between the belt and the roller. Employee #1 climbed up the ladder to get the wood. The conveyor pulled in his hand and took his arm off. The conveyor was running with a speed of about 50 to 60 feet per minute throughout the entire maintenance procedure. He yelled "Turn it off, turn it off." He was taken to North Broward Medical Center and later airlifted to Broward General Hospital.
