Investigation Summary
Employee #1 was working along the leading edge of a single-story low-slope roof without the benefit of any type of fall protection. Employee #1 fell approximately 13 ft 6 in. to a solid concrete inclined surface (driveway). Employee #1's right frontal lobe was fractured sufficiently to allow cerebrum discharge. Employee #1 was taken to the hospital where he died on January 1, 2003.
