Investigation Summary

Investigation Nr: 200622413
Event: 05/17/2003
Employee killed when truck crushes him

%09On May 17, 2003, Employee #1 was driving a Dump Truck on East Division %09Street in Mt. Juliet, TN. The truck's left front tire veered off of the road and the employee corrected the situation by turning the steering wheel back to the left. He over compensated, and the truck swerved to the left, ejecting him out of the passenger side window. The truck continued in a swerve to the left and fell on its right side. It continued sliding until it reached the ditch on the left side of the street, where it then began to roll over. Once the truck got caught in the drainage ditch, it was stopped in its roll over motion and fell back down on its right side, trapping the employee, as he lay on the road, beneath the right front wheel. East Division Street is a very narrow road, only 22 ft, 6 in. wide. There were no approaching vehicles. The employee was not wearing his seat belt. Employee #1 was killed.
