Investigation Summary
On April 17, 2003, Employee #1 was holding a tag line that was attached to crank-up scaffold frames. The frames were suspended from the forks of a rough-terrain forklift. The scaffold frames began to sway as the forklift moved forward, causing the forklift to fall on its left side. The carriage at the end of the boom hit the employee in the back, causing multiple body injuries.
