Employee falls from the ladder and dies

On March 15, 2003, Employee #1, of Golden Inn, was assisting a contracted Air Conditioning Technician servicing the cooler tower located at the middle of the low-pitched roof of the motel. While accessing a portable ladder for a decent, Employee #1 fell 17 ft from the roof of a two-story building. He was killed. The 12-foot long aluminum, single-section lean-to ladder had been placed upon the upper landing of an outdoor stairwell which had 42-in.-tall handrails and was 113 in. above the asphalt paved parking lot. The upper landing of the stairwell was graded metal and measured 60 in. by 60 in. The unsecured footing of the ladder had been placed close to the edge of the top step of the stairwell and the upper portion of the ladder rested upon the rain gutter of the roof's eave located 22 in. horizontally over and 92 in. above the upper stairwell landing.
