Machine Traps Employee Between Guard And Control Panel

At approximately 1:20 a.m. on August 6, 2003, Employee #1 was working in Core Room 1. Employee #1 was caught between the Redford 23 control panel and the guard behind the core box. While the machine was in operation and the box was closed the distance measured 14 in. wide. The depth from the cylinder to the outer cover was 9 in. wide. The core making operation was activated with two-handed controls. However, once the box closed with the core inside, the operator could remove his hands from the controls. Employee #1 was caught at the point when the core box opened. Employee #1 was found in an upright position with his back against the control panel and his chest against the box guard. Employee #1 sustained a chest injury with fractured ribs and a pneumo-thorax (accumulation of air or gas in the pleural cavity) and was hospitalized.
