Investigation Summary
On April 21, 2003, Employee #1 was preparing to move power poles that were secured to racks on top of a trailer. He released four straps that were holding in two 40 ft long wood utility poles when the butt end of one of the poles rolled off the rack. This caused the other end of the pole to swing up, striking Employee #1 in the head. Employee #1 was killed.
