Investigation Summary
On February 17, 2004, Employee #1, a masonry laborer, was working on a construction site that required the installation of brick on the mezzanine of a building. The mezzanine was 12 feet above the ground. Employee #1 approached a fork lift operator and requested that the operator lift both Employee #1 and a buggy of mortar to the mezzanine area. Two sheet of plywood were placed across the forks of the forklift, which Employee #1 and the buggy were on top of. As the operator lifted Employee #1 and the buggy to the mezzanine, Employee #1 stepped off the plywood to the floor of the mezzanine and attempted to pull the buggy off the plywood. The buggy began to roll to one side of the plywood. Employee #1 grabbed a hold of the buggy, stepping on a section of unsupported plywood. He lost his balance and fell to the ground. Employee #1 was hospitalized and treated for injuries sustained.
