Employee killed when struck by mechanical power press

On April 8, 2003, Employee #1 was behind a Cleveland brand 400-ton mechanical power press. The operator observed Employee #1 on the back side and it was unknown what he was doing at the rear side. The operator observed Employee #1 fall behind the press as it was being operated. The mesh guard was in the up position on the rear side where Employee #1 apparently got his head struck by parts on the press, receiving skull fractures and a laceration across his jaw. Employee #1 was transported to the hospital where he died three days later from his injuries.
