Employee killed after falling from bridge

On March 5, 2003, Employee #1, as part of three man crew, was installing steel I-beams and other hardware to reinforce the support structure of a bridge over a river. The crew was working on a suspended scaffold, under the bridge. Employee #1 was wearing a safety harness and lanyard tied off to the scaffold. Another crew member was tied off to the horizontal lifeline supplied for tie off and the foreman was not tied off at all. The scaffold suspension line (line one of four) failed, causing the entire scaffold to fall approximately 50 to 60 ft into the river. The crewmember tied to the lifeline did not fall to the river and the foreman fell clear of the scaffold. Employee #1 fell into the river with the scaffold and was pinned beneath the scaffold in the water. Employee #1 was killed.
