Employee Struck And Crushed By Press

Employee #1 was discussing the placement of a new back-up bearing press with her supervisor. After a brief discussion the supervisor left the area. Employee #1 was apparently pulling the back-up press toward herself to put it in position for possible use. The press weights 1300 pounds and is over 6 ft high. The frame is 24 in. wide by 30 in. deep and has four caster wheels. Employee #1 was pulling the press, and it tipped over onto her when the front wheels started to go over a metal floor grate trough cover. The wheels apparently swiveled and grabbed causing the press to tip. Employee #1 was transported to the hospital where she was pronounced dead.
