Employee caught between scissor lift guardrail and wall

On November 13, 2002, Employee #1 and a coworker were working from a scissor lift with a height capability of 20 feet. The employees needed a scissor lift with higher reach potential. They decided to obtain one from another part of the facility. They selected a Skyjack SJIII Series, Model Number 4830, battery-powered, self-propelled scissor lift. To drive the lift to the work area, both employees entered the scissor lift platform. The coworker was positioned approximately in the middle of the platform, and Employee #1 was positioned at the rear of the platform against the guardrail, in relation to the direction of travel. The coworker began driving the lift toward the wall opening in the lift's reverse travel mode. The coworker faced the direction of travel, while reaching behind to operate the lift's controls. With the lift positioned in the wall opening, but neither employee through the opening, the coworker advised Employee #1 that they were going through the opening, and to get down. The coworker continued driving the lift through the wall opening at an angle, then stopped and travelled in the opposite direction for approximately 1 foot to reposition the lift to avoid contact with the opposite side of the hallway, which they were travelling into. The coworker then returned in the direction of travel into the hallway adjacent the wall opening. After the coworker was through the wall threshold, he heard a bump sound that he thought was making contact with something. The coworker turned to see that Employee #2's hard hat had fallen to the floor and that Employee #1's head was caught between the scissor lift platform guardrail's top rail and the wall system at the top of the wall opening. Employee #1 was hospitalized as a result of his head injuries and died 15 days later.
