Investigation Summary
On February 4, 2004, Employee #1, a 16-year-old employee, was using a saw to remove a catalytic converter from a junk car at a salvage yard. While Employee #1 was under the car, a floor jack holding up the car slipped or failed and fell on Employee #1. Employee #1 was killed. There were no other blocking means to secure the car.
