Employee is killed by a falling forklift

On June 2, 2003, Employee #1, a forklift operator, was loading pallets of computers into a trailer when the truck went forward and a forklift fell between the dock and the trailer. Employee #1 sustained a fracture of his head and was killed. He was not wearing his seat belt and the trailer was not chocked.
