Employee Killed In Fall From Scaffold

On March 31, 2003, Employee #1 was working on a 42 foot high scaffold. The employee kneeled down working from the work platform. The employee began to rise from the kneeled position. He used the guardrail to assist him in rising from the kneeled position. However, the guardrail was unstable and gave way, causing Employee #1 to fall to the ground. Employee #1 died at the scene.
