Employee Falls 14 Feet Onto Hoist Platform

On October 31, 2002, Employee #1 was loading his tools onto a material hoist platform. While backing into the material hoist shaft, the interlocking hardware, which prevented the doors from opening until the platform was in position, did not function correctly. The hoist platform was at the bottom of the shaft. Employee #1 fell 14 feet onto the material hoist platform below, fracturing his arm. He was hospitalized.
