Investigation Summary
On February 10, 2005, an employee was part of three person sheet metal roofing team that was installing sheet metal on a mansard using 20-ft fiberglass extension ladder. The employee stated that when the ladder was positioned 2 hours prior to the accident, the concrete surface was covered with ice and 2 inches of snow. The employee stated that the ladder feet were positioned in the stake manner as opposed to using the pads on the bottom of the feet so that the ladder wouldn't slide on the ice. When the snow and ice melted, the metal stakes on the feet slid on the concrete. The ladder was secured to the building using a bungee cord, which stretched and then gave way as the ladder slid away from the building. This caused the employee to slide along with the ladder. The employee stated that as the ladder fell, he grabbed for the edge of the roof and then fell, landing on his feet. The compression fracture to the vertebrae occurred due to the pressure of landing on his feet from 12 feet, and the fracture to the right arm occurred when his arm struck the edge of the roof as he fell. The employee was hospitalized and treated from his injuries.
