Employee Dies after Fall from Ladder

Employee #1 was working at a residential construction roofing site replacing an existing roof structure. He was working from a 20-foot extension ladder and was removing materials from the edge of the roof when the ladder slid away from the structure. He fell approximately 8 feet 7 inches to the ground and struck his head. He died nine days after the accident as a result of complications arising from a subdural hematoma. The investigation revealed that the employee had not received training on the proper use of the ladder or the dangers associated with shifting weight while on an extension ladder.
