Employee injured after falling from forklift work platform

On March 23, 2004, an employee was getting parts out of a storage rack with the use of a Gopher forklift. The forklift had its forks extended 12 to 18 in. into the aisle running south into the facility. A coworker on a Raymond stand-up forklift turned into the isle the employee's lift was in and struck the forks of the lift. The lift was turned side-ways in the isle and caused the employee to fall off of the work platform of the lift and strike the concrete floor. The employee was hospitalized and treated for broken lower right leg.
